What this page covers
This page explains how we handle personal information collected through this website, mainly the enquiry, referral and feedback forms. Information collected once support begins is covered separately in your service agreement, which we go through with you before anything is signed.
What we collect through this site
Only what you type into a form: your name, an email address or phone number, the suburb and postcode you mention, the support you are interested in, your funding arrangement if you tell us, the role of a referrer where relevant, and your message.
We ask you not to include health records, diagnoses, medication or other sensitive information in these forms. If we need that information later, we ask for it directly and through a secure channel.
Why we collect it
To read your message, work out whether we can help, and reply to you. Nothing sent through this site is used for marketing, sold, or shared with third parties for their own purposes.
Who can see it
Messages sent through this site are stored so that only our intake team can read them. They are not publicly readable and cannot be listed or retrieved by visitors to the site.
Where support proceeds, information is shared only with the people who need it to provide that support. Anything beyond that is discussed with you first, except in the rare circumstances where the law requires disclosure, and we tell you when that happens.
How long we keep it
Enquiries that do not lead to support are kept only as long as needed to answer them and to show that we responded, and are then deleted. Records connected to supports we provide are kept for the periods required by law and by NDIS practice standards.
Access, correction and complaints
You can ask what we hold about you, ask for it to be corrected, and ask for a copy. Send that request through the contact form and tell us how you would like to receive the answer.
If you are unhappy with how we have handled your information, tell us. You can also raise a privacy complaint with the Office of the Australian Information Commissioner, and a concern about an NDIS provider with the NDIS Quality and Safeguards Commission.
